Totem of holding mysterious bug

Hello, my friends and I have recently had some trouble with the quark totem of holding on our modded server. Sometimes when we die a totem not only didn't spawn but also our items were simply nowhere to be found, well before they would have despawned and far away from anything like lava that could have destroyed them. My friend experienced this when he died to a mob and I experienced this when testing using /kill, but not consistently. When it did happen for me, a message popped up in chat saying "an unextpected error occured when trying to execute that command" after using /kill. My efforts to replicate the bug have all failed, I have no idea what causes it. anyone know anything about this? any solutions or causes?

May be that the player took damage twice and died twice somehow?
I have a grave mod in my server and there are some things that hit twice and don't check if the player's already dead, so the grave drops items as soon as the player dies the second time.
